Shift: 8-hour days, 5 days per week
Preferred Schedule: Tuesday–Saturday
Alternative Schedule: Monday–Friday
Weekend Requirement: Every other weekend or Tuesday–Saturday coverage

Position Overview

Seeking an experienced Home Health RN to care for medically complex adult patients recently discharged from the hospital. Many patients are at elevated risk for readmission and require skilled assessment, education, care coordination, and close follow-up.

Required Qualifications

• Current RN license
• Current BLS
• Valid driver’s license
• Active automobile insurance
• Home Health experience strongly preferred
• HomeCare HomeBase experience preferred
• Must be comfortable managing wounds, wound VACs, IV therapy, and PICC lines

Common Patient Diagnoses

• Congestive heart failure
• COPD
• Complex wounds
• Wound VACs
• IV therapy
• PICC line management
• Post-hospitalization care

Productivity Expectations

Daily productivity goal is 5.25 points, generally equal to approximately 5 visits per day.

• Start of Care: 2.5 points
• Resumption of Care: 1.75 points
• Recertification: 1.5 points
• Routine visit: 1 point

Daily visit volume may be lower when Start of Care visits are scheduled due to their higher point value.
