We are seeking an experienced and motivated Referral Coordinator III to join our healthcare team in Fairbanks, Alaska. This position plays a critical role in ensuring patients receive timely, coordinated, and high-quality care by managing referrals between primary care providers, specialty clinics, external healthcare organizations, and patients throughout the region.

As a key member of a multidisciplinary care team, you will serve as the communication hub connecting patients, providers, specialists, and community healthcare partners. You will help improve patient outcomes through effective referral management, care coordination, telehealth support, and patient advocacy.

Position Summary

The Referral Coordinator III supports Primary Care Teams and Specialty Clinics by coordinating medical referrals within and outside the healthcare system. This position ensures referrals are accurately processed, authorized, tracked, and completed while maintaining regulatory compliance and exceptional customer service.

In addition to referral coordination responsibilities, this role facilitates telehealth and video teleconferencing visits between patients and remote healthcare providers.

The successful candidate will demonstrate advanced knowledge of referral coordination, healthcare systems, and patient care processes while serving as a resource and mentor to other team members.

Key Responsibilities

Referral Coordination & Patient Care

  • Coordinate referrals for preventive, chronic, specialty, and acute care services.
  • Collaborate with primary care and specialty providers to actively manage patient referrals.
  • Initiate, monitor, and track referrals with internal and external healthcare partners.
  • Verify and process referral authorizations, including Purchased/Referred Care (PRC) and Veterans Administration (VA) referrals.
  • Ensure timely and accurate documentation within the Electronic Health Record (EHR).
  • Maintain HIPAA compliance and strict patient confidentiality.
  • Manage a closed-loop referral process, ensuring providers receive referral outcomes and follow-up documentation.
  • Coordinate all referral types, including specialty clinics, regional medical centers, local providers, and VA services.

Patient & Care Team Communication

  • Educate patients and families regarding referral processes and next steps.
  • Serve as a liaison between patients, providers, specialists, and care teams.
  • Maintain regular communication through phone, email, electronic systems, and in-person interactions.
  • Support continuity of care through proactive coordination and follow-up.

Telehealth & Video Teleconferencing Support

  • Prepare patients for telehealth appointments.
  • Obtain vital signs and assist with telehealth diagnostic equipment, including ECGs and digital otoscopes.
  • Serve as a tele-presenter during virtual consultations with remote specialists.
  • Coordinate specialist recommendations and required follow-up care.

Leadership & Professional Development

  • Demonstrate flexibility and composure in a fast-paced healthcare environment.
  • Act as a resource, mentor, and positive role model for team members.
  • Assist with onboarding and training of referral coordinators and clinical staff.
  • Provide coverage for the Lead Referral Coordinator as needed.
  • Participate in quality improvement initiatives, audits, chart reviews, and process improvement projects.
  • Assist in reviewing, revising, and documenting procedures, workflows, and policies.

Clinical & Operational Support

  • Greet and escort patients to support efficient clinic flow.
  • Assist with patient transport as needed.
  • Monitor exam room readiness and medical equipment functionality.
  • Support ancillary departments when needed, including family medicine, pediatrics, surgery, orthopedics, and specialty clinics.

Minimum Qualifications

Required

  • Current Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) credential.
  • Five (5) years of healthcare experience, including at least two (2) years of referral coordination experience;

OR

  • Completion of an approved training program and seven (7) years of referral coordination experience.
  • Current Basic Life Support (BLS) certification or ability to obtain within 30 days of hire.
  • Strong understanding of healthcare systems, referral management, and care coordination.
  • Ability to maintain strict confidentiality and comply with HIPAA requirements.
  • Ability to pass required background screening.

Preferred

  • Alaska Native or American Indian applicants are encouraged to apply.
  • Experience working within Tribal Health, Indian Health Service (IHS), or comparable healthcare systems.
  • Additional certifications such as:
    • Phlebotomy
    • Medical Coding
    • EMT
    • Wound Care
    • Surgical Technology
    • BLS Instructor
    • Other healthcare-related certifications or degrees

  • Proficiency in medical terminology and Electronic Health Records (EHR).
