Travel Physical Therapist (Home Health) – Fort Smith, AR
Job Details
- Job Title: Travel Physical Therapist
- Location: Fort Smith, AR & surrounding areas
- Setting: Home Health
- Schedule: Monday–Friday | 8:00 AM – 4:30 PM
- Weekly Hours: 32 Hours Guaranteed
- On-Call/Weekend: No regular on-call; schedule flexibility may be required to support patient care, including occasional weekend rotation or observed holidays as needed.
- Mileage Reimbursement: Yes (between patient visits)
- Dress Code: Royal Blue Scrubs
Job Responsibilities
- Perform comprehensive physical therapy evaluations, including assessment of functional status, body systems, and rehabilitation needs in accordance with CMS and state guidelines.
- Develop individualized treatment plans in collaboration with the physician and interdisciplinary healthcare team.
- Provide evidence-based physical therapy interventions to reduce pain, improve mobility, restore function, and maximize patient independence.
- Monitor patient progress and communicate changes in condition to the physician and nursing team.
- Educate patients, families, and caregivers on treatment plans, home exercise programs, safety, and functional limitations.
- Complete evaluations, progress notes, visit documentation, discharge summaries, and all required clinical documentation accurately and within agency timelines.
- Travel throughout the assigned service area to provide in-home patient care.
- Supervise Physical Therapist Assistants (PTAs) and Home Health Aides.
- Participate in interdisciplinary meetings, staff conferences, quality improvement initiatives, and continuing education.
- Maintain compliance with all agency, state, federal, CMS standards of practice.
Required Qualifications
- Active Physical Therapist (PT) License in the state of Arkansas (or compact eligibility if applicable).
- Home Health experience is required.
- OASIS documentation experience is required.
- Current CPR/BLS Certification.
- Valid driver's license, reliable transportation, and willingness to travel throughout the service area.
- Strong clinical assessment, documentation, communication, and patient education skills.
Preferred Qualifications
- Experience with Homecare Homebase (HCHB) EMR.
