GENERAL DESCRIPTION: Provides professional clinical care for transplant candidates, recipients, living donors and their families.
Pre Transplant Coordination
- In the pre transplant phase a transplant coordinator works closely with potential renal recipients to coordinate the evaluation process and provide education in preparation for transplantation.
- Ensures financial screening was completed prior to scheduling patients for transplant evaluation
- Reviews medical records of potential recipients scheduled to attend information session and create complication list
- Educates potential pre-transplant recipients regarding the required pre-transplant testing, physical and psychosocial exams.
- Assists in scheduling transplant work-up appointments
- Reviews potential recipients lab work and pre-evaluation tests with transplant physician overseeing the evaluation
- Assists transplant physician in presenting the patients case along with the rest of the team to determine patient’s suitability for listing or to determine further work up is needed
- Assures that protocol guidelines and physician orders are carried out as prescribed
- Completes accurate and thorough documentation to facilitate patient care, and meet compliance and reimbursement requirements
- Draws pre-transplant labs for potential recipients
- Assists in coordinating transplant; call in potential recipient (if deceased donor transplant), or schedule recipient (living donor transplant)
- Shares transplant call with other transplant nurse coordinators
- Documents on call patient discussions and any changes in patient medications
- Performs patient telephone triage, assesses needs, and coordinates needs with medical staff.
- Reviews waitlist monthly with transplant team members in patient selection meeting
- Evaluates patients annually to ensure each candidate continues to meet selection criteria
- Educates staff on the transplant process
