Description
Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop and implement individualized treatment plans to improve strength, balance, mobility, and independence.
- Provide therapeutic exercises, gait training, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs and safety techniques.
- Document patient evaluations, treatments, and progress using Epic Remote Client, Rover, and OASIS.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers.
- Maintain productivity expectations while delivering quality patient care.
Requirements
- Minimum 1 year of recent Skilled Home Health Physical Therapy experience required.
- First-time travelers accepted.
- OASIS experience is required.
- Current Physical Therapist license (Pending Oregon license accepted).
- Current BCLS certification.
- Valid driver's license, reliable personal vehicle, and current auto insurance required.
- Must have completed the COVID-19 primary vaccination series plus one booster.
