Description
Physical Therapist (Home Health)
Requirements:
- Minimum 1 year of Home Health PT experience
- OASIS experience/training preferred
- BLS certification required
- Valid Driver's License and reliable vehicle required
- COVID vaccination (or approved exemption) required
- Flu vaccine or declination (mask required if declined)
- Must be able to complete 6–8 home visits/day
- Weekend rotation: 1–2 weekends per contract
- Experience with Medicare/Skilled Home Health patients preferred
