Position Overview
The Social Worker provides support to patients and their families/significant others to help them adjust to illness, injury, hospitalization, recovery, or end-of-life situations. This role collaborates closely with the interdisciplinary healthcare team to address social, emotional, economic, and family-related factors that impact patient care and outcomes. The Social Worker is responsible for facilitating safe, timely, and appropriate discharge planning, coordinating transitions of care, and connecting patients with community resources and support services.
Key Responsibilities
Discharge Planning
- Participate in and implement discharge planning services for patients.
- Coordinate referrals to nursing homes, assisted living facilities, board and care homes, group homes, hospice services, and other post-acute care settings.
- Validate discharge criteria and ensure readiness for transition to the next level of care.
- Follow up with patients, families, and receiving facilities to support successful care transitions.
- Identify and address social, environmental, and psychosocial barriers that may impact discharge planning.
Assessment & Intervention
- Conduct comprehensive psychosocial assessments, including evaluation of prior functioning, support systems, coping abilities, and risk factors.
- Provide intervention and support in cases involving child abuse, neglect, domestic violence, guardianship, foster care, adoption, mental health placement, and adult or elder abuse.
- Assist patients and families in managing the emotional, social, and financial impacts of illness, injury, or disability.
- Connect patients and families with appropriate community resources, support programs, and services.
- Serve as a resource regarding palliative care, advance care planning, treatment decisions, and end-of-life issues.
Care Coordination
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and implement patient-centered care plans.
- Coordinate services and communicate discharge plans with community agencies and post-acute providers.
- Ensure continuity of care through accurate documentation and effective communication across care settings.
- Advocate for patients and families to ensure access to appropriate services and support systems.
Required Qualifications
- Bachelor’s or Master’s degree in Social Work from an accredited program.
- Active Licensed Social Worker (LSW) license in the State of Minnesota.
- Minimum of 2 years of hospital social work experience.
- Demonstrated experience with discharge planning in an acute care setting.
- Experience working with patients across all populations and age groups.
- Proficiency with Epic electronic medical records (EMR) system.
- Strong knowledge of community resources, healthcare regulations, and care coordination practices.
- Excellent communication, assessment, problem-solving, and interpersonal skills.
Preferred Skills
- Experience supporting complex psychosocial and discharge planning cases.
- Knowledge of post-acute care placement processes and community-based services.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
- Strong crisis intervention and advocacy skills.
- Experience working with palliative care and end-of-life planning.
Required Candidate Submission Materials
- Current resume with complete work history.
- Skills checklist relevant to social work practice, completed within the last year.
- Two professional supervisory references, including employment dates.
- Documentation of active Minnesota LSW licensure.
- Disclosure of U.S. work authorization status, if applicable.
Schedule Requirements
- Participation in every seventh weekend rotation.
- No on-call responsibilities.
