Emergency Department (ER/ED) Unit Overview

Facility Capabilities

• Trauma Level: IV Trauma Center
• STEMI Receiving: Yes, but STEMIs requiring intervention transfer out (no interventional cath lab)
• Stroke Receiving: Yes; strokes requiring intervention transfer out
• Beds:
  • 24 ED rooms
  • 4 low-acuity bays with chairs (used as Fast Track)
  • 1 trauma/crash room within main ED
• Fast Track: Yes — utilizes low-acuity bays
• Average Daily Volume: 110–120 patients/day
• RN Ratios: 1:3–4

ED Layout & Patient Flow

• Separate triage area operating 24/7
• Travelers typically do not perform triage
• ED holds are intermixed with the department
  • Boarding is uncommon
  • Longest hold experienced: ~20 hours
  • If boarding reaches 3–4 patients, Med/Surg RN support is brought down
• Very fast room turnover:
  • Minimal downtime
  • Discharged patients are quickly replaced with new arrivals
  • Lobby protocols may be started while patients wait (IVs, labs, orders)

Behavioral Health

• 4 dedicated behavioral health rooms within the ED
• Rooms have safety features:
  • “Garage door” style closures to secure supplies, monitors, and equipment
• Sitters available as needed

Security

• In-house security 24/7
• Security station located in lobby next to triage

Patient Population

All ages served:

• ~20% pediatric patients (<18)
• Pediatric patients requiring higher-level care transfer to Boise
• Common presentations:
  • Respiratory issues
  • Abdominal pain
  • Stroke symptoms
  • STEMI/chest pain
  • Outdoor injuries (hiking, fishing hooks, etc.)
  • Farming/agricultural injuries
  • Walk-in trauma
• Diverse population including:
  • Migrant workers
  • Farming community
  • Older rural residents

Required Experience & Certifications

Experience:

• Minimum 1–2 years ED experience required
• First-time travelers accepted
• Must be comfortable managing ED flow
• Must have pediatric comfort level

Required Certifications:

• BLS
• ACLS
• PALS
• TNCC
• ENPC

Providers

• 7 ED physicians staffed over a 24-hour period
• Mid-level coverage:
  • Noon–Midnight
  • 2 AM–8 AM: one MD only

Nursing Support Staff

• Charge RN:
  • Available 24/7
  • Does not take patient assignments
  • Works 7 AM–7 PM
• Support staff:
  • ED techs
  • Patient sitters as needed
  • Unit secretary 24/7

Respiratory Therapy

• RT available 24/7
• 2–3 RTs in-house at all times
• Performs:
  • ABGs
  • Respiratory treatments
• Manages:
  • Ventilators
  • CPAP
  • BiPAP
  • High-flow oxygen
• Nurses assist mainly with troubleshooting

Diagnostics

• X-ray and CT located in the same hallway as ED
• Transport handled by imaging staff
• Labs:
  • RNs primarily draw blood
  • Phlebotomy assists with difficult sticks
• EKGs performed by:
  • RN staff
  • ED techs

Monitoring

• No dedicated ED monitor techs
• RNs interpret their own telemetry strips

Scheduling

Shift Length: 12-hour shifts

• 0730
• 0900
• 1000
• 1130
• 1330
• 1530
• 1930
• Self-scheduling available
• Final schedule adjusted by manager for balance

Weekend/Holiday Requirements

• Every other weekend
• Holidays only if staffing needs require

• None

Possible float opportunities:

• Meridian ED
• Boise ED
  (rare occurrence)

Dress Code

• Any color scrubs
• Must be clean and professional

Orientation

• 1 unit-specific ED orientation shift
