Description
THIS IS A LOCAL CONTRACT
Seeking qualified Social Worker who has experience in both medical and skilled nursing settings. This will be a "jack-of-all trades" type role that can include Rehab, hospital visits, home visits, case management, and outaptient clinical duties.
MUST have 1+ year working with Geriatric populations
MUST have 2+ years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferable
Masters in Social Work is STRONGLY preferred
