Travel Clinical Care Leader RN – Post-Acute Care

Start Date: ASAP
Contract Length: 13 weeks
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Shift Length: 8 hours
Guaranteed Hours: 40 hours per week
Weekend Requirement: On-call coverage approximately every 5th weekend
Holiday Requirement: Required; schedule to be determined
Charting: PointClickCare
BSN Required: No
Overtime: Available

Position Overview

Seeking an experienced RN Clinical Care Leader for a post-acute setting. This role combines direct resident care, clinical oversight, interdisciplinary coordination, staff delegation, documentation, and unit leadership.

Key Responsibilities

• Complete comprehensive resident assessments
• Develop, implement, and evaluate individualized care plans
• Coordinate care with providers and the interdisciplinary team
• Monitor changes in resident condition and respond appropriately
• Maintain accurate documentation in PointClickCare
• Administer and reconcile medications
• Delegate tasks to unlicensed staff based on competency and scope
• Supervise assigned clinical personnel
• Maintain accountability for resident assignments
• Support quality-improvement initiatives, committees, and department projects
• Ensure care complies with applicable clinical standards and regulations

Required Qualifications

• Active RN license
• Current BLS
• Recent post-acute, skilled nursing, long-term care, or similar experience
• Strong assessment, care-planning, delegation, and leadership skills
• Comfortable managing a resident assignment while providing clinical oversight

Preferred Experience

• Resident Assessment Instrument process experience
• MDS or long-term care assessment experience
• PointClickCare experience

Scheduling and Call

• Standard schedule is 8:00 AM–4:30 PM
• On-call coverage approximately every 5th weekend
• Holiday rotation is required and will be determined by the facility
• Workweek runs Sunday through Saturday
