Temp-to-Perm Opportunity | $46/hour

Position Details

• Pay: $46/hour
• Schedule: Monday-Friday, 8:00 AM-5:00 PM
• Assignment: Temp-to-Perm
• Full-Time: 40 hours per week

What You'll Do

• Coordinate and manage care plans for participants with complex healthcare needs
• Participate in interdisciplinary team (IDT) meetings and collaborate with healthcare professionals
• Review clinical documentation for quality and compliance
• Coach and support field staff to ensure high standards of care
• Coordinate services including home care, rehabilitation, nursing home placement, transportation, and other community resources
• Monitor participant progress and modify care plans as needed
• Collaborate with physicians, specialists, discharge planners, and other providers to ensure continuity of care
• Maintain accurate documentation in accordance with regulatory and organizational standards
• Assist with staff training, competencies, and quality improvement initiatives

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program
• Current, active New York State Social Work license
• Minimum of 2 years of care management or healthcare management experience
• Experience in home care, managed care, long-term care, hospital, or related healthcare setting preferred
• Experience working with the elderly or frail population preferred
• Strong communication, organizational, and documentation skills
• Supervisory or leadership experience is a plus
• Bilingual candidates are encouraged to apply
