Description

Job Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) under the supervision of a Registered Nurse (RN) or Director of Nursing (DON). The LPN is responsible for medication administration, resident assessments, treatments, documentation, and supervising Certified Nursing Assistants (CNAs) while ensuring compassionate, high-quality resident care. LPNs in LTC settings play a central role in monitoring residents' health, coordinating care, and maintaining compliance with facility policies and regulatory standards.

Key Responsibilities

  • Administer prescribed medications, injections, IV therapy (if permitted), and treatments.
  • Perform resident assessments and monitor changes in physical and mental condition.
  • Record vital signs and accurately document care in the Electronic Medical Record (EMR).
  • Develop, implement, and update individualized care plans.
  • Provide wound care, catheter care, and other skilled nursing procedures within scope of practice.
  • Respond promptly to medical emergencies and notify physicians/RNs of significant changes.
  • Supervise, delegate, and support CNAs and nursing assistants.
  • Educate residents and families regarding care plans, medications, and health conditions.
  • Ensure compliance with CMS, OSHA, HIPAA, infection control, and facility policies.
  • Collaborate with physicians, therapists, social workers, and interdisciplinary healthcare teams.
  • Maintain accurate documentation and participate in quality improvement initiatives.

Submission Requirements

  • 3+ years current exprience (must be within the last 6 moths) - [REQUIRED]
  • Must be willing to float where experienced - [REQUIRED]
  • IV certified (must be willing to obtain during onboarding if not certified) - [REQUIRED]
  • All RTO must be on initial presentation for approval and can not exceed 5 days (Holidays NOT approved) -[REQUIRED]
  • Covid Vaccination Required - Declinations/Exemptions are not accepted (Please upload proofs with upload) - [REQUIRED]
  • Locals residing within 50 miles of the facility will not be accepted (Please ensure perm address is in profile) - [REQUIRED]
  • Previous travel experience – [Highly Preferred]
  • Rural/small facility experience – [Highly Preferred]
