Description

Position Overview

We are seeking a dedicated professional to join a dynamic healthcare team in a skilled nursing facility. The candidate will play a crucial role in maintaining the well-being and comfort of residents and will collaborate with healthcare staff to ensure comprehensive care.

Responsibilities

Perform comprehensive assessments of patients' conditions and develop individualized care plans.
Monitor patients' progress and adjust care plans as necessary.
Foster an environment of compassionate care.
Engage in effective communication with patients, families, and the health care team.
Utilize critical thinking skills to address patients’ healthcare needs.
Adhere to facility policies and procedures and maintain compliance with all regulatory standards.

Qualifications

Highly organized with excellent communication and interpersonal skills.
Able to work effectively in a fast-paced environment.
Strong critical thinking and problem-solving skills.
Ability to maintain confidentiality and privacy regarding patient information.
Flexibility and adaptability to changing patient needs.

Experience

Previous experience in a skilled nursing facility or similar setting preferred.
Proficient in using healthcare technologies.
