Job Description
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our integrated care team in Bronx, NY. In this hybrid role, you will provide comprehensive behavioral health services, supporting clients through assessments, treatment planning, crisis intervention, and ongoing care coordination. Responsibilities
- Conduct comprehensive psychosocial assessments through client interviews and record reviews.
- Develop, implement, and monitor individualized treatment plans.
- Provide individual and group counseling services.
- Track client progress and adjust treatment goals as needed.
- Collaborate with interdisciplinary care teams to coordinate patient care.
- Facilitate referrals to community resources and healthcare providers.
- Perform crisis assessments and provide appropriate interventions.
- Deliver wellness-focused and integrated behavioral health services.
- Maintain accurate, timely, and confidential clinical documentation.
Qualifications
- Master's degree in social work (MSW) from an accredited program.
- Active New York Licensed Clinical Social Worker (LCSW) license.
- 1–2 years of clinical mental health experience preferred.
- Strong assessment, counseling, and crisis intervention skills.
- Excellent communication and documentation abilities.
- Bilingual in English and Spanish is preferred.
