Schedule:

  • 7 days on / 7 days off
  • Shift: 5:00 PM – 8:00 AM

Position Overview:
Join our dedicated team in a rewarding after-hours role providing high-quality patient care and clinical support. This position offers a balanced 7-on/7-off schedule, allowing for extended time off while making a meaningful impact on patients during evenings and overnight hours.

Key Responsibilities:

  • Provide after-hours triage by responding promptly to patient and caregiver calls, assessing needs, and determining the appropriate level of care.
  • Collaborate closely with the inpatient interdisciplinary team to ensure seamless communication and continuity of care.
  • Complete after-hours admissions, including comprehensive patient assessments, documentation, and development of individualized care plans.
  • Conduct necessary patient visits to address urgent clinical needs, symptom management, and changes in condition.
  • Perform overnight check-in calls with patients admitted between 8:00 AM and 5:00 PM to assess their status, answer questions, reinforce education, and provide ongoing support.
  • Accurately document all patient interactions and interventions in the electronic medical record while maintaining compliance with organizational policies and regulatory standards.
  • Serve as a compassionate clinical resource for patients, families, and caregivers, ensuring exceptional care throughout the overnight hours.
