Job Summary

Under the direction and supervision of the Wound and Ostomy Manager, the Wound, Ostomy, and Continence Nurse (WOCN) provides safe, competent, and high-quality patient care based on evidence-based nursing practice, clinical expertise, and current research. The WOCN delivers specialized care to patients with complex wound, ostomy, continence, fistula, and skin integrity needs.

Responsibilities

  • Conduct comprehensive health assessments focused on wound, ostomy, and continence care. Perform focused physical assessments, develop individualized plans of care based on patient diagnoses and needs, and recommend appropriate treatment interventions.
  • Assist with the development, implementation, revision, and evaluation of standards of care, clinical guidelines, and best practices.
  • Assess, monitor, and document patient responses to wound care, ostomy, continence, fistula, and complex wound management interventions. Evaluate the quality and effectiveness of care delivered by clinical staff.
  • Collaborate with interdisciplinary healthcare teams and initiate referrals to appropriate services and specialists as needed.
  • Educate, counsel, and support patients and families in managing ostomy care, wound prevention, continence issues, and strategies to prevent complications.
  • Provide comprehensive assessments for patients at risk of developing pressure injuries and initiate evidence-based prevention strategies.
  • Participate in unit, department, and hospital-wide Quality Improvement (QI) initiatives, including incidence and prevalence studies, nursing quality projects, and performance improvement activities.
  • Provide culturally competent education to patients, families, and staff while respecting cultural, ethnic, religious, and individual belief systems.
  • Maintain the security, privacy, and confidentiality of patient information in accordance with organizational policies and regulatory requirements.
  • Utilize electronic medical record systems, clinical equipment, and applicable healthcare and billing systems effectively.
  • Perform other duties and responsibilities as assigned.
  • Must be willing to float to other areas as needed.
  • Required: Wound care certification.

Required Skills & Experience

  • Three to five (3–5) years of healthcare experience required.
  • Experience providing specialized wound, ostomy, and continence care preferred.
  • Ability to effectively utilize BHMC electronic health record systems, clinical equipment, and other healthcare-related systems relevant to the position.
  • Strong interpersonal, communication, and collaboration skills.
  • Requirements are in addition to all RN contracted labor requirements.

Required Certifications & Licensure

  • Active New York State Registered Nurse (RN) license.
  • One of the following certifications required:
    • Certified Wound Ostomy Nurse (CWON) issued by the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB); or
    • Certified Wound Care Nurse (CWCN) issued by the WOCNCB; or
    • Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or
    • Wound Care Certified (WCC) credential issued by the National Alliance of Wound Care and Ostomy (NAWCO).
