Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation:

Local Pay: $55.00 - $70.00/hour (W-2)

Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend) *

Location: Sylva, NC, 28779

Schedule details (hours per day/week): 32 hours/week guarantee

Productivity expectations: 30 points

On Call/Weekend Requirement: None

Coverage Areas: Jackson and Swain Counties NC

Shift schedule (days, start & end times): 8:00 AM to 04:30 PM

Key Responsibilities:

•  Conduct HH PT SOC, evaluations, reassessments, discharges, and routine visits as needed in patient homes within Jackson/Swain counties in NC.
• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
• Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.

Must have qualifications/experience: NC PT License, OASIS experience
