Description
Submission Requirements
- Minimum of 2+ years of hospital Social Work experience (Required)
- Experience with discharge planning (Required)
- Experience working with diverse patient populations (Required)
- Active Licensed Social Worker (LSW) license in the State of Minnesota (Required – Documentation Required)
- Epic Electronic Medical Record (EMR) experience (Required)
Profile Requirements
- Complete work history (Required)
- Current skills checklist relevant to the specialty, completed within the last 12 months (Required)
- Two supervisory references with employment dates included (Required)
- Candidate work authorization status must be clearly documented if not eligible to work in the USA
- All requested time off (RTO) must be submitted at the time of candidate submission. NMH may not approve RTO requests submitted after submission.
State License Requirements
