Shift: 5x8 Hour Day Shifts (Monday–Friday, 8:00 AM–5:00 PM)
Contract Length: 13 Weeks
Start Date: August 31, 2026 (ASAP considered)
Schedule: Day Shift with rotating on-call responsibilities
 

Job Summary

We are seeking an experienced Registered Nurse (RN) – Home Health to provide skilled nursing care to patients in their homes. The RN will deliver compassionate, patient-centered care in accordance with physician orders and individualized plans of care while working under the supervision of the Director of Clinical Services/Nursing Supervisor.

The ideal candidate will have recent home health experience, be proficient with Homecare Homebase (HCHB), and possess hospice experience.
 

Responsibilities

• Deliver skilled nursing care in patients' homes according to physician orders and established care plans.
• Complete Start of Care (SOC), recertification, resumption of care, and routine skilled nursing visits.
• Assess, monitor, and document patient conditions accurately and promptly.
• Educate patients, families, and caregivers on disease management, medications, treatments, and home care.
• Coordinate care with physicians and interdisciplinary healthcare team members.
• Maintain accurate documentation using Homecare Homebase (HCHB).
• Participate in scheduled on-call rotations and provide after-hours patient support as assigned.
• Travel throughout the designated service area to provide patient care.

Coverage Area

• Nelson County
• Spencer County
• Southern Bullitt County

Travel within the assigned territory is required. Mileage is not reimbursable.

Patient Population

• Adult patients
• Pediatric patients
• Hospice patients

Required Qualifications

• Active Registered Nurse (RN) license
• Current BLS Certification
• Valid driver's license and proof of automobile insurance
• Recent Home Health nursing experience (Required)
• Homecare Homebase (HCHB) experience (Required – No Exceptions)
• Hospice experience (Required)

Schedule & Call Requirements

• Monday through Friday, 8:00 AM–5:00 PM
• Every third weekend on-call rotation
• One on-call weeknight each week
• Clinicians are responsible for contacting patients to coordinate visit times.

Additional Information

• Home Health Aides available for patient support
• No floating required
• No utilization review responsibilities
• No point-based productivity system
• Any color scrubs permitted
• BSN not required
• Guaranteed 40 hours per week
• Facility may cancel up to four shifts during a 13-week assignment in accordance with contract policy.
