Registered Nurse (RN) – Home Health & Hospice
 

Shift: 8-Hour Day Shifts (Monday–Friday, 8:00 AM–4:30 PM)
Contract Length: 13 Weeks
Schedule: Days with rotating weekends, on-call, and holiday coverage
 

Job Summary

We are seeking an experienced Registered Nurse (RN) – Home Health & Hospice to provide skilled nursing care to patients in their homes. This dual-role position offers the opportunity to care for both home health and hospice patients while collaborating with an interdisciplinary team to deliver compassionate, high-quality care.

The RN will assess patients, develop and implement individualized care plans, perform skilled nursing procedures, educate patients and caregivers, and ensure accurate documentation in accordance with physician orders and agency policies.
 

Responsibilities:

• Provide skilled nursing care to home health and hospice patients in their homes.
• Complete comprehensive patient assessments and develop individualized plans of care.
• Perform routine nursing visits, admissions, and case management responsibilities.
• Deliver patient and caregiver education regarding disease management, medications, and treatment plans.
• Perform skilled procedures including:
  • Wound care and wound VAC management
  • PICC line dressing changes
  • Port access
  • IV infusions
  • Catheter insertions
  • Drain management
  • Laboratory specimen collection
• Complete accurate OASIS documentation and maintain timely electronic medical records.
• Collaborate with physicians, LPNs, case managers, and the interdisciplinary care team to ensure continuity of care.
• Participate in scheduled on-call, weekend, holiday, and after-hours coverage as assigned.

Patient Assignment

• Routine visits: Approximately 5–6 patients per day
• Admissions: Approximately 2 per day
• RN case load: Approximately 20–25 patients
• RNs work closely with an LPN, with the RN visiting each assigned patient weekly.

Required Qualifications

• Active Registered Nurse (RN) license
• Current BLS Certification
• Valid driver's license and reliable transportation
• Minimum of 1.5 years of RN experience
• Home Health experience required
• Hospice experience preferred (required for first-time travelers)
• OASIS documentation experience
• Strong clinical assessment and critical thinking skills
• Experience with infection control practices, wound care, wound VACs, PICC lines, IV infusions, catheter insertion, drain management, and port access
• Ability to travel throughout the assigned service area, including nighttime driving when needed

Schedule & Call Requirements

• Monday–Friday, 8:00 AM–4:30 PM
• Every third weekend rotation
• Up to one 12-hour overnight on-call shift per week (8:00 PM–8:00 AM)
• Holiday coverage required when scheduled

Orientation & Work Environment

• Field-based orientation
• Charting completed in Epic Remote
• Office located in Rapid City, South Dakota
• No floating required
• Free parking available
• Scrubs of any color or business casual attire permitted

Additional Requirements

• Travelers must reside more than 100 miles from any Monument Health facility.
• Two professional references from the past 12 months are required.
• Previous Monument Health employees must wait one full year before returning as a traveler.
• First-time travelers will be considered if they have prior Home Health and Hospice experience.
