Registered Nurse (RN) Case Manager – Home Health
Locations: Bradford, VT | Randolph, VT | Springfield, VT | White River Junction, VT | Lebanon, NH
Shift: Full-Time | Days
Contract: 16 Weeks
Weekly Gross: $2825
Mandatory Qualifications for this Job
- Active Vermont RN, New Hampshire RN, or Compact RN License
- Graduate of an Accredited Nursing Program (ADN or BSN)
- 2+ years of Home Health RN experience
- Current BLS Certification
- Home Health Case Management experience
- Proficiency with EPIC EMR
- Strong patient assessment, care planning, and documentation skills
- Excellent communication and teamwork skills
- Valid driver's license and reliable transportation (preferred)
- Ability to work independently and manage a home health caseload
Roles and Responsibilities
- Conduct comprehensive patient assessments in the home setting
- Develop and update individualized care plans
- Coordinate care with physicians and the interdisciplinary team
- Monitor patient progress and report changes in condition
- Provide skilled nursing care, medication management, and patient education
- Educate patients and caregivers on disease management and self-care
- Maintain accurate documentation in EPIC EMR
- Participate in care conferences and discharge planning
- Promote patient safety, independence, and quality care
- Ensure compliance with home health policies and regulatory standards
