ER RN Unit Notes
Unit Overview
- Department: Emergency Room (ER)
- Beds: 45 total
- 4 hallway beds
- 4 vertical (fast-track) rooms
- 1 trauma bay
- 1 resuscitation bay
- 1 cardiac room
- 4 psychiatric rooms (included in total bed count)
Trauma & Specialty Designations
- Trauma Level IV (currently accepting Level III trauma patients while pursuing Level III designation)
- STEMI Level I
- Stroke Level II
Patient Population
- Adults and Pediatrics
- High-acuity emergency department
- Common diagnoses:
- Stroke
- STEMI
- Trauma
- Abdominal pain
- ESI Level I/high-acuity patients
- Pediatric patients are stabilized and transferred as needed.
Patient Volume & Staffing
- Average daily census: ~170 patients/day
- RN:Patient Ratio: 1:4–5
- CNA Ratio: Approximately 1:5–9
Experience Requirements
- Minimum 2.5 years of Emergency Department experience
- Experience in a larger urban hospital
- Trauma experience preferred
Required Certifications
- BLS
- ACLS
- TNCC
- PALS
- ENPC
Emergency Department Layout
- Fast Track via Vertical Space/Rapid Assessment
- Separate triage area
- Travelers do not perform triage
- ER holds are intermixed with active ER patients
- 4 dedicated psych rooms with 1:1 sitters
- Security stationed in the ER 24/7
Providers
- Minimum staffing:
- 1 Emergency Physician
- 1 APP
- Up to 6 providers may be staffed simultaneously.
Nursing Support
- Charge Nurse (no patient assignment)
- Assistant Nurse Manager
- Administrative Supervisor
- CNAs perform:
- EKGs
- Phlebotomy
- Wound care (no irrigation)
- Providers perform splinting.
Respiratory Therapy
- RT available 24/7
- Performs:
- ABGs
- Breathing treatments
- RT manages all respiratory equipment.
Monitoring & Diagnostics
- RNs interpret telemetry
- SpaceLabs cardiac monitors
- X-ray and CT located in the ER
- Overflow imaging in main Radiology
- Patients transported unless critically ill
- Labs drawn by:
- RNs
- CNAs
- Phlebotomy
Common Medication Drips
- Nicardipine
- Propofol
- Fentanyl
- Norepinephrine
- Nitroglycerin
- Versed (Midazolam)
- Precedex (Dexmedetomidine)
Scheduling
- 12-hour shifts with varying start times:
- 7:30 AM–8:00 PM
- 10:00 AM–10:30 PM
- 11:30 AM–12:00 AM
- 1:30 PM–2:00 AM
- 3:30 PM–4:00 AM
- 7:30 PM–8:00 AM
- Mixed start times based on department needs
- No block scheduling
- No late-to-early ("turnaround") shifts
Weekend & Holiday Requirements
- 4 Saturday/Sunday shifts every 6 weeks (2 full weekends)
- Holidays as needed
- No call or standby requirement
Floating
- Rare
- May float to:
- Boise Medical Center ER
- Nampa Medical Center ER
Orientation
- Hospital orientation
- Epic orientation
- 1 unit-specific shift in the ER
Dress Code
- Any clean, professional scrubs
- No required scrub color
Facility Policy
- 50-mile radius rule applies for travelers.
