We are currently seeking a compassionate and motivated Registered Nurse (RN) to join our growing team.

Key Responsibilities

• Perform initial and ongoing patient assessments to determine skilled nursing needs

• Complete Start of Care (SOC), Resumption of Care (ROC), Recertifications, and Discharges accurately and timely

• Provide skilled nursing services including wound care, IV therapy, injections, catheter care, and medication management

• Educate patients and caregivers on disease processes, care plans, and safety protocols

• Coordinate care with physicians, therapists, social workers, and interdisciplinary team members

• Serve as Case Manager for assigned patients (as applicable)

• Document all care accurately in OASIS-E using EMR (WellSky)

• Supervise and document care provided by LVNs and Home Health Aides (HHAs)

• Participate in case conferences, team meetings, and quality improvement activities

• Ensure compliance with Medicare Conditions of Participation and California Title 22 regulations

Qualifications

• Active and unrestricted California RN license

• Graduate of an accredited nursing program

• Minimum 1 year of RN experience 

• Current CPR certification

• Valid California driver’s license, reliable transportation, and active auto insurance

• Strong clinical assessment, communication, and critical thinking skills

• Ability to work independently in a field-based setting

• Bilingual skills are a plus
