Licensed Practical Nurse (LPN) – Long-Term Care | New York

Assignment Details

• Contract Length: 13 Weeks
• Start Dates: Every 2 weeks (based on orientation schedule)
• Available Shifts:
  • 5×8-Hour: Days & Evenings
  • 3×12-Hour: Nights (subject to staffing approval)
• Important: Shift cannot be changed once an offer is accepted.

Position Requirements

• Minimum 1 year of Long-Term Care (LTC) or Skilled Nursing experience
• Experience caring for Alzheimer's & Dementia patients
• First-time travelers accepted
• Active New York State LPN License
  • No license hits accepted
  • Proof of NY license is required at the time of submission
• BLS Certification

Schedule & Facility Requirements

• Patient Ratio: 1:18–20
• Every other weekend required (based on facility needs)
• Local candidates accepted
• Floating: Not required
• Block scheduling: Not available
• Night shift is generally a slower-paced unit.

Time-Off Policy

• RTO must be submitted at the time of application.
• No additional RTO or shift changes will be approved after an offer is made.
• Requests to have every holiday off will not be approved.

Responsibilities

• Administer medications and prescribed treatments
• Perform medication passes accurately and safely
• Monitor and report changes in resident condition
• Complete timely and accurate documentation
• Provide quality nursing care according to facility policies
• Supervise CNAs on assigned units

Additional Requirements

• Must successfully pass:
  • Medication Administration (Med Pass) Test
  • Clinical Observation
• Strong attendance and reliability are required.
• Any call-outs must be communicated more than 24 hours in advance.
• Compliance process takes approximately 1.5–2 weeks.

Required Training

Once your offer is accepted, you'll receive access to Relias training modules within 48 hours via email. All required modules must be completed no later than 3 days before your assignment start date.

Hard Stops

• ✅ Active NY State LPN License (proof required at submission)

• ✅ RTO must be submitted before the offer is made
• ✅ Must pass Med Pass test and clinical observation
