Job Description
Position: Medical Social Worker (Critical Care)
Shift:
- Day Shift: Monday–Friday, 8:30 AM – 5:00 PM
- 14 weeks contract.
Responsibilities:
- Provide social work services for patients in the Medical Intensive Care Unit (MICU).
- Perform early identification and high-risk/readmission screening for patients.
- Provide counseling to help patients adjust to illness.
- Conduct safety assessments.
- Coordinate and facilitate discharge planning.
- Identify and address obstacles and barriers to safe discharge.
- Collaborate with the interdisciplinary healthcare team to ensure continuity of care.
Requirements:
- Recent hospital experience in Critical Care units.
- Experience using Epic EMR.
- Active Maryland LMSW or LCSW-C license or an equivalent active out-of-state social work license.
- Candidates with an equivalent out-of-state license must have a completed Maryland licensure application on file with the Maryland Board to be eligible to work.
Schedule:
- Monday–Friday
- 8:30 AM – 5:00 PM
Guaranteed Hours:
- Guaranteed hours as specified by the contract.
