Description
MUST HAVE: CANDIDATE MUST HAVE A WOUND CARE CERTIFICATE.
Shift: Days - 5x8
Job Summary:
-Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.
Responsibilities:
-Conducts health assessments focused on wound, ostomy, and continence related care. Performs focused physical assessment, develops plan of care based on the diagnosis and individual client needs. Recommends treatments as appropriate.
-Assist with the development, revisions, and evaluations of standards of care.
-Assess and document patient response to wound care, ostomy, continence, fistula and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff.
-Works collaboratively with the health care team and makes referrals to other disciplines as indicated.
-Teach, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems.
-Routinely providing comprehensive assessments of patients at risk for developing pressure injuries. Initiating preventative measures.
-Participate in Unit/Department Quality Improvement and Hospital Wide Q.I activities, i.e. Incidence, Prevalence Studies and Nursing Quality.
-Provide culturally competent patient, family and staff education consistent with cultural, ethnic, religious and other belief systems.
-Maintain the security and confidentiality of data and information in accordance with patient rights policies.
-Performing other tasks as assigned.
-Must be willing to float as needed.
-MUST HAVE: Wound care certificate
Required Skills & Experience:
-Three to five (3-5) years’ experience in healthcare.
Required Certifications & Licensure:
-Active NYS Registered Nurse license.
-Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB; or Certified Wound Care Nurse (CWCN) issued by the (WOCNCB); or Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
