Clinical Support Unit – Float Pool (Labor & Delivery)

Charting System

Unit Overview

Department: Clinical Support Unit – Float Pool (Women's Services)

Float Pool Coverage

RNs float only within Women's Services and may receive full assignments in:

  • Antepartum
  • Labor & Delivery (L&D)
  • Postpartum

Float assignments may occur at any regional facility:

  • Boise
  • Meridian
  • Nampa

Staffing & Patient Ratios

  • Follows AWHONN staffing guidelines
  • Postpartum couplet care ratio: 1 RN : 6 couplets

Required Certifications

  • BLS
  • ACLS
  • NRP
  • AWHONN Fetal Monitoring
  • STABLE preferred (required for permanent staff)

Patient Population

Antepartum

  • 22+ weeks gestation
  • High-risk pregnancies

Labor & Delivery

Common diagnoses include:

  • PPROM
  • Placenta previa
  • Preeclampsia
  • Diabetes
  • Multiple gestation (twins)
  • Other high-risk obstetric patients

Postpartum

  • Full couplet care assignment
  • Car seat studies
  • Mother/baby transition when NICU staff are unavailable

Required Experience

Must Have

  • Labor & Delivery experience
  • Antepartum experience
  • Postpartum experience
  • Minimum 3 years of experience
  • Excellent communication skills
  • Ability to utilize chain of command and available resources

Preferred

  • OR circulating for Cesarean sections

Not Accepted

  • First-time travelers

Nursing Responsibilities

Drips

RNs are required to titrate drips.

Common drips include:

  • Pitocin (typically 1–20 units; may increase up to 40 units per protocol)
  • Insulin
  • Magnesium sulfate

Unit Support

Charge Nurse

  • Present on every shift and every unit

Support Staff

  • Surgical Techs
  • Occasional Patient Care Techs (less common on nights)

Respiratory/Neonatal Support

  • PPV performed as needed
  • NICU team attends deliveries
  • OB Laborist available 24/7
  • Neonatologist available 24/7 (at least one regional site with rapid response capability)
  • Dedicated CRNA in-house 24/7

Monitoring

  • No telemetry technicians on the unit
  • Cardiac telemetry strips are interpreted by Tele Techs
  • Fetal monitoring performed per unit practice

Scheduling

Shift Times

  • Days: 06:45–19:30
  • Nights: 18:45–07:30
  • Schedule: 4 × 12-hour shifts

Scheduling Notes

  • No self-scheduling
  • Shift trades permitted
  • Manager may adjust schedule as needed
  • Request time off before assignment begins

Weekend Requirement

  • Every other weekend
  • Travelers work unit norms but no more than every other weekend

Holidays

  • Travelers generally not scheduled
  • Occasional holiday coverage may be required

Call

  • No call or standby requirement

Floating Process

  • Travelers are first to float
  • May float:
    • Between Women's Services units
    • Between Boise, Meridian, and Nampa facilities
    • Mid-shift if census requires
  • Assignment available through scheduling app approximately 1 hour before shift
  • Facilities are approximately 20 minutes apart
  • Easy interstate access and safe parking at all sites

Orientation

Approximately 4–5 orientation shifts, including:

  • 1 shift – Nampa (L&D/Postpartum workflow)
  • 1 shift – Meridian
  • 1 shift – Boise
  • Partial shift – Boise Antepartum
  • Orientation includes L&D, Antepartum, Postpartum, and OR workflows

Dress Code

  • Hospital-provided scrubs

Culture & Expectations

  • Strong teamwork and supportive culture
  • Leadership available at all times, although managers are not always physically on-site
  • Travelers are expected to:
    • Communicate proactively
    • Check email regularly
    • Use the chain of command appropriately
    • Know available resources
    • Advocate for patients and communicate concerns promptly
