Submission Requirements

• 2 years current FIRST ASSIST OPERATING ROOM experience [Required]
• Bachelor of Science degree  [Required]
• BLS AHSA  [Required]
• CNOR REQUIRED   [Required]
• Meditech Expanse experience [highly preferred]
• Perm address must be included in profile
• Traveler must have their own vehicle for this need
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION.

FACILITY WILL NOT CONSIDER ANY TRAVELER WHO RESIDES WITHIN 50 MILES OF FACILITY.  To avoid delays in processing, please include perm address at profile upload.

• Work History
• Skills Checklist, FIRST ASSIST OPERATING ROOM, current to 1 year [REQUIRED] [DOCUMENT]
• 1 Supervisory Signed Reference (from within the past two years of employment) [REQUIRED]
• Massachusetts RN license - Must be uploaded with profile [REQUIRED] [DOCUMENT]
• Active American Heart Association BLS - [REQUIRED] [DOCUMENT]
• Active Certified Perioperative Nurse CNOR - [REQUIRED] [DOCUMENT]

NOTE:  FACILITY WILL NOT PEND CERTS.
