Description

Registered Nurse (RN) – Home Health

Location: Lexington, KY 
(Coverage: Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas & Woodford Counties)
Agency: Commonwealth Home Health – Lexington
Schedule: Monday–Friday | 8:00 AM–4:30 PM
Guaranteed Hours: 32+ hours/week
Productivity: 30 points/week
On-Call: 1 weekend every ~4 weeks + 5–6 weekdays/month
Mileage: Reimbursed between patient visits  

 

Position Summary

Seeking an experienced Home Health RN to provide skilled nursing care, complete OASIS documentation, manage a multi-county caseload, and collaborate with an interdisciplinary team to deliver high-quality patient care.

Key Responsibilities

  • Provide skilled nursing assessments, treatments, and patient education.
  • Perform SOC, ROC, Recertification, Follow-Up, and Discharge visits.
  • Complete accurate OASIS documentation within required timelines.
  • Develop and update individualized care plans.
  • Collaborate with physicians, therapists, and care teams.
  • Meet productivity goals while maintaining quality and regulatory compliance.

Required Qualifications

  • Active Kentucky RN license.
  • 1–2 years of Home Health experience.
  • Homecare Homebase (HCHB) experience required.
  • Strong OASIS knowledge.
  • Current BLS certification.
  • Valid driver's license and reliable transportation.

Preferred Skills

  • Excellent clinical judgment, time management, territory management, and communication skills.
  • Strong problem-solving abilities with a team-first approach.
