Description

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

Travel Pay: $2,460 gross per week

Location: Grand Rapids, MI, 49546

Shift: 8 Hours/Day, Monday to Friday, 09:00 AM to 05:00 PM
Guaranteed Hours: 40 Hours/Week
Contract Duration: 13 Weeks

Key Responsibilities:

Conduct comprehensive home health assessments, complete OASIS documentation, develop individualized Plans of Care (POC), and provide skilled nursing interventions to ensure quality patient care.
Coordinate care with physicians, interdisciplinary teams, patients, and families while delivering education on medications, disease management, home safety, infection prevention, and self-care.
Perform skilled nursing procedures including wound care, IV therapy, venipuncture, catheter and drain management, pain management, and other home health clinical interventions.
Maintain accurate clinical documentation, supervise Home Health Aides (HHA/LVN/STNA), ensure compliance with regulatory and payer requirements, and participate in interdisciplinary meetings and on-call responsibilities as assigned.

Required Qualifications

Active Registered Nurse (RN) license in the state of practice.
Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
Minimum 2+ years of Home Health experience.
Minimum 1–2 years of RN clinical experience.
Homecare Homebase (HCHB) experience – Required.
OASIS documentation experience – Required.
Current CPR/BLS Certification.
Valid driver's license.
Reliable transportation with current auto insurance.
Ability to travel locally for patient visits.
