Description
Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.
Compensation:
- Travel Pay: $2,460 gross per week
Location: Grand Rapids, MI, 49546
- Shift: 8 Hours/Day, Monday to Friday, 09:00 AM to 05:00 PM
- Guaranteed Hours: 40 Hours/Week
- Contract Duration: 13 Weeks
Key Responsibilities:
- Conduct comprehensive home health assessments, complete OASIS documentation, develop individualized Plans of Care (POC), and provide skilled nursing interventions to ensure quality patient care.
- Coordinate care with physicians, interdisciplinary teams, patients, and families while delivering education on medications, disease management, home safety, infection prevention, and self-care.
- Perform skilled nursing procedures including wound care, IV therapy, venipuncture, catheter and drain management, pain management, and other home health clinical interventions.
- Maintain accurate clinical documentation, supervise Home Health Aides (HHA/LVN/STNA), ensure compliance with regulatory and payer requirements, and participate in interdisciplinary meetings and on-call responsibilities as assigned.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
- Minimum 2+ years of Home Health experience.
- Minimum 1–2 years of RN clinical experience.
- Homecare Homebase (HCHB) experience – Required.
- OASIS documentation experience – Required.
- Current CPR/BLS Certification.
- Valid driver's license.
- Reliable transportation with current auto insurance.
- Ability to travel locally for patient visits.
