Job Summary

Caledonia Home Health & Hospice, a division of Northern Counties Health Care, is seeking a compassionate and dedicated Home Health & Hospice RN to provide skilled nursing care to patients of all ages in their homes. The RN will perform patient assessments, admissions, care planning, and collaborate with an interdisciplinary team to deliver coordinated, patient-centered care.

Required Qualifications

• Current Vermont RN License (Required)
• Minimum 1 year of clinical nursing experience
• Home Health or Hospice experience preferred
• Excellent communication, assessment, and care coordination skills
• Ability to work independently and as part of a multidisciplinary team

Key Responsibilities

• Perform patient admissions, assessments, and ongoing evaluations.
• Develop, implement, and update individualized care plans.
• Provide skilled nursing care to patients across the lifespan.
• Coordinate care with physicians, therapists, and home health professionals.
• Educate patients and caregivers on disease management, medications, and home care.
• Maintain accurate documentation and ensure compliance with home health standards.
• Deliver compassionate, high-quality care while promoting patient independence and safety.
