Description
Job Description – Nurse Practitioner (NP) | PACE Program
Weekly Pay: $4946
Location: Grand Junction, CO
Contract: 13 Weeks
Schedule: 4 x 10-hour Day Shifts (On-site)
Responsibilities
- Provide primary care services for elderly patients enrolled in the PACE program.
- Diagnose and treat acute and chronic medical conditions.
- Perform preventive care, health assessments, and urgent care visits.
- Evaluate patients in the clinic, PACE Day Center, and in their homes as needed.
- Collaborate with the Medical Director and interdisciplinary care team.
- Coordinate referrals to specialists and community providers.
Requirements
- Minimum 2 years of Nurse Practitioner experience.
- Experience in Primary Care, Geriatrics, Family Practice, Internal Medicine, or Hospice/Palliative Care preferred.
- Active NP license and prescriptive authority (per state requirements).
- Strong clinical assessment and patient management skills.
