Description
Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.
Compensation:
- Local Pay: $52.00/hour (W-2)
Location: Phoenix, AZ, 85014
- Shift: 8 Hours/Day, Monday to Friday, 09:00 AM to 05:00 PM
- Guaranteed Hours: 40 Hours/Week
- Contract Duration: 14 Weeks
Key Responsibilities (KRAs):
- Conduct comprehensive home health and/or hospice assessments, develop and manage individualized Plans of Care (POC), perform skilled nursing interventions, complete OASIS documentation, and coordinate patient care with physicians, interdisciplinary teams, patients, and families to ensure quality outcomes.
- Provide patient and caregiver education, maintain accurate clinical documentation, supervise home health aides/LVNs, ensure compliance with infection control and payer requirements, participate in on-call rotation, and support continuity of care through effective communication and case management.
Required Certifications, Licenses & Skills:
- Required Licenses/Certifications: Active State Registered Nurse (RN) License, Current CPR Certification, Valid Driver's License with reliable transportation and auto insurance. Preferred: CHPN (Certified Hospice and Palliative Nurse) Certification, BSN.
- Required Skills & Experience: Minimum 2 years of Home Health RN experience, proficiency with Homecare Homebase (HCHB) and OASIS documentation, strong clinical assessment, wound care, IV therapy, venipuncture/phlebotomy, catheter/drain management, patient education, case management, and the ability to work independently while traveling locally.
