Home Health Registered Nurse (RN)

As a Home Health RN, you will provide skilled nursing care to patients in their homes, helping them maintain independence and improve quality of life.

Position Details:

• Full-Time
• 8-Hour Day Shifts
• EMR: Homecare Homebase
• Average caseload: 5-6 patients per day

Covered Territories:

• Norwalk
• Stamford
• Greenwich
• Ridgefield
• Wilton
• Redding

Hospice Registered Nurse (RN)

As a Hospice RN, you will provide compassionate end-of-life care and support to patients and families in the community.

Position Details:

• Full-Time
• 8-Hour Day Shifts

Covered Territories:

• Milford
• Washington
• Kent
• Litchfield
• Norwalk
• Greenwich

Requirements

• Active Registered Nurse (RN) License
• Current Resume
• Completed Skills Checklist
• Home Health and/or Hospice experience preferred
