Position Overview

We are seeking an experienced and motivated Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Shelby County and portions of Fayette and Tipton Counties. This field-based role is responsible for patient assessments, care planning, OASIS documentation, ongoing case management, and collaboration with physicians and interdisciplinary team members to ensure high-quality patient outcomes. The ideal candidate will be OASIS proficient, self-directed, and committed to delivering exceptional care in the home setting.

Schedule & Work Details

• Coverage Area: Shelby County, portions of Fayette County, and Tipton County
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hours: 40 hours
• Pay Rate: $51.00 - $53.00 per hour
• Productivity Expectation: 30 points per week
• On-Call Requirement: Possible on-call rotation every 4 weeks
• Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies.
• Conduct comprehensive patient assessments and ongoing evaluations.
• Complete OASIS assessments and documentation accurately and timely.
• Develop, implement, and update individualized plans of care.
• Monitor patient progress and communicate significant changes in condition to physicians and appropriate care team members.
• Coordinate care with therapists, aides, physicians, and other interdisciplinary team members.
• Educate patients and caregivers on disease management, medications, treatments, and home safety.
• Maintain accurate and compliant clinical documentation.
• Ensure adherence to Medicare, state, federal, and agency regulations.
• Manage daily travel within the assigned coverage territory while delivering patient-centered care.
• Participate in agency meetings, case conferences, and quality improvement initiatives as required.

Required Qualifications

• Active Registered Nurse (RN) license.
• Current CPR certification.
• OASIS proficiency required.
• Valid driver's license and reliable transportation.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Ability to work independently and effectively manage a field-based caseload.

Preferred Qualifications

• OASIS Certification.
• Previous home health nursing experience.
• Knowledge of Medicare home health regulations and documentation requirements.
• Strong organizational and time management skills.
• Experience coordinating care across interdisciplinary teams.
