Licensed Physical Therapy Assistant (LPTA) – Home Health
Job Type: Contract
Duration: 13 Weeks
Schedule: 8-Hour Day Shifts
Guaranteed Hours: 40 Hours/Week
Position Summary
We are seeking an experienced Licensed Physical Therapy Assistant (LPTA) to provide home health physical therapy services under the supervision of a Licensed Physical Therapist. The LPTA will implement treatment plans, monitor patient progress, document care, and collaborate with the interdisciplinary team to help patients improve mobility, function, and independence.
Key Responsibilities
- Administer physical therapy treatments as directed by the supervising Physical Therapist.
- Perform therapeutic exercises, manual therapy, and physical modalities, including heat, cold, massage, traction, and electrical stimulation.
- Monitor patient progress and accurately document treatments in Homecare Homebase (HCHB).
- Educate patients and caregivers on therapy techniques and home exercise programs.
- Coordinate patient schedules and communicate changes with the care team.
- Follow infection prevention, safety, and regulatory guidelines.
- Participate in interdisciplinary meetings, in-services, and quality improvement initiatives.
Required Qualifications
- Active Licensed Physical Therapy Assistant (LPTA) license in the practicing state.
- Minimum 2 years of Physical Therapy Assistant experience.
- Homecare Homebase (HCHB) experience required.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Associate Degree from an accredited Physical Therapy Assistant program.
Preferred Qualifications
- Previous Home Health experience.
- Strong communication, documentation, and time management skills.
- Ability to work independently and manage a home-based patient caseload.
- Physical ability to lift, bend, push, pull, and stand for extended periods while providing patient care.
