REQUIRED: - ACTIVE License - LMSW or LCSW AND ACTIVE MS State License- Minimum 2 years of Acute Inpatient Medical Case Management experience that includes at least 1 year (in the last 3) in a LARGE BUSY Hospital ED (500+ beds, LEVEL 1/ACADEMIC Medical Center) - Recent (within the last two years) Acute care discharge planning experience for patients with a wide range of medical diagnosis is REQUIRED, - Heavy experience in Psychosocial assessments, telephonic, utilization management/review, insurance company experience, rehab, LTC, Hospice,
