LPN/LVN – Long-Term Care (LTC) / Skilled Nursing Facility

Location: Vestal, NY 

Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide compassionate nursing care in a State-run Long-Term Care facility serving residents requiring skilled nursing, rehabilitation, memory care, and wound care services. The ideal candidate will have prior LTC/Skilled Nursing experience, excellent medication administration skills, and the ability to work effectively in a fast-paced team environment.

Schedule

• 13-Week Contract
• 5x8-Hour Shifts (40 Hours/Week)
• Available Shifts:
  • Evening Shift: 2:45 PM – 11:15 PM (14:45 – 23:15)
  • Night Shift: 11:45 PM – 7:15 AM (23:45 – 07:15)
• Start dates available every two weeks (based on orientation schedule)
• Every other weekend required
• Shift cannot be changed once offered
• 12-hour shifts may be available with prior staffing approval

Requirements

• Active New York State LPN License (Required)
  • No license hits accepted
  • Proof of license required at the time of submission
• Minimum 1 year of Long-Term Care (LTC) or Skilled Nursing Facility experience
• First-time travelers accepted
• BLS Certification (Required)
• Must successfully complete a Medication Administration (Med Pass) Test and Clinical Observation
• Strong attendance, reliability, and punctuality required

Patient Population

• Alzheimer's Disease

Patient Ratio

• Approximately 1:18–20
• 300-bed Skilled Nursing Facility

Responsibilities

• Administer medications and treatments as prescribed
• Perform accurate and safe medication passes
• Monitor residents and report changes in condition
• Complete timely and accurate documentation
• Provide nursing care in accordance with facility policies
• Supervise Certified Nursing Assistants (CNAs)
• Maintain resident safety and quality of care
• Collaborate with the interdisciplinary healthcare team

Important Notes

• No shift changes or additional RTO will be approved after an offer is accepted.
• RTO must be submitted with the application.
• Cannot request every holiday off.
• Any call-outs must be communicated at least 24 hours in advance.
• Compliance process takes approximately 1.5–2 weeks.
• Relias training modules will be sent within 48 hours of offer acceptance and must be completed no later than 3 days before the assignment start date.
