Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN/LVN) to join a 41-bed Short-Term Rehabilitation Unit specializing in orthopedic recovery, cardiac rehabilitation, wound care, IV therapy, and nephrostomy care. The ideal candidate will have experience in Skilled Nursing or Rehabilitation settings, be proficient with Epic charting, and provide compassionate, patient-centered care while collaborating with the interdisciplinary healthcare team.

 

Schedule

  • 13-Week Contract
  • Shift: Varies (Days/Nights)
  • Weekend Requirement: Every Other Weekend (EOW)
  • Floating: Yes, based on facility needs
  • First-Time Travelers Accepted: Yes
  • Travelers must reside more than 75 miles from the facility (Locals not accepted).

Requirements

  • Minimum 1 year of Skilled Nursing Facility (SNF) or Rehabilitation experience
  • Epic charting experience required
  • BLS Certification (Required)
  • Patient ratio experience of 1:10
  • Ability to float to other units as needed

Patient Ratios

  • Days: 1:8
  • Nights: 1:20
  • Weekends: 1:10

  • Congestive Heart Failure (CHF)

Responsibilities

  • Perform comprehensive patient assessments and admissions
  • Administer medications and treatments safely
  • Monitor vital signs, intake/output, and laboratory results
  • Complete medication reconciliation and discharge planning
  • Provide patient and family education
  • Perform wound care, dressing changes, and wound vac management
  • Assist with rehabilitation and mobility care, including ambulation and orthopedic recovery
  • Manage feeding tubes, Foley catheters, ostomy care, and blood glucose monitoring
  • Administer oxygen therapy and provide respiratory support as needed
  • Monitor cardiac and renal patients and report changes in condition
  • Maintain accurate documentation using Epic EMR
  • Collaborate with physicians and the interdisciplinary care team
  • Float to other units based on patient care needs
