Physical Therapist - Physical Therapist at West North HH - CA (Redding, CA)
Start ASAP for 13 weeks Schedule: Monday-Friday
Coverage: Opal (Cottonwood south to Orland) May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address. Average 4-6 visits per day depending on type of visit for PT/OT/SLP Productivity is 28-32 points per week for PT/OT/SLP SOC: 2.5 points, ROC: 2 points,
Evaluations/Recert/Discharge OASIS: 1.5 points, Discipline DC, routine visit, 30-day reassessment: 1 point, Weekly case conference: 1 point Any overtime must be pre-approved. 2 weeks cancellation Home Health and Oasis experience required, no exceptions Homecare Homebase documentation experience required.
