Description
Registered Nurse (RN) – Home Health/Hospice
Location: Fort Mill, SC
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Shift: 8 Hours | Guaranteed Hours: 40/Week
Pay Range : $46/hr - $51/hr on W2
Position Summary
Seeking an experienced Registered Nurse (RN) to provide skilled nursing care to home health and hospice patients in their homes. The RN will assess patients, coordinate care with the interdisciplinary team, develop and manage care plans, and ensure quality patient outcomes.
Responsibilities
- Perform patient assessments and identify physical, psychosocial, and environmental needs.
- Develop, implement, and evaluate individualized Plans of Care (POC).
- Provide skilled nursing care while maintaining accurate documentation.
- Educate patients and families on medications, safety, diet, and self-care.
- Coordinate care with physicians, families, and healthcare teams.
- Supervise HHA/STNA/LVN staff to ensure quality care delivery.
- Perform wound care, IV care, venipuncture, catheter/drain care, and other nursing procedures as required.
- Ensure compliance with infection control, regulatory standards, and payer requirements.
- Participate in interdisciplinary meetings and on-call duties as assigned.
Required Qualifications
- Current RN license in the practicing state.
- Graduate of an accredited nursing program (ADN/Diploma required; BSN preferred).
- Current CPR certification.
- Homecare Homebase (HCHB) and OASIS experience required.
- 1–2 years of clinical RN experience and 2+ years of home health experience required.
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to travel locally for patient visits.
Work Environment
Field-based home health/hospice role requiring travel to patient homes in and around Cheraw, SC. On-call responsibilities may be required.
