Monday–Friday
5x8-hour shifts
8:00 AM–4:30 PM

Paid hourly, not per visit
Start-of-care visits and revisits
Requirements:
- Recent and relevant home health physical therapy experience
- EPIC experience
- OASIS experience
- Start-of-care and revisit experience
- Active Physical Therapist license
- BLS certification
