Description

Registered Nurse (RN) – Home Health/Hospice

Location: Grand Rapids, MI
Pay Range: $50–$55/hr
Shift: Monday – Friday, 9:00 AM – 5:00 PM
Guaranteed Hours: 40 Hours/Week
Contract Length: 14 Weeks

Position Summary

The Registered Nurse (RN) provides skilled nursing care to home health and hospice patients in their place of residence. The RN assesses patient needs, develops and implements individualized Plans of Care (POC), coordinates care with the interdisciplinary team, and ensures quality patient care.

Responsibilities

  • Assess home health and hospice patients and identify physical, psychosocial, and environmental needs.
  • Develop, implement, document, and evaluate individualized Plans of Care (POC).
  • Maintain accurate patient documentation and clinical records.
  • Educate patients and families on medications, home safety, self-care, and disease management.
  • Supervise HHA/STNA/LVN staff and coordinate care with the interdisciplinary team.
  • Assess and manage patient pain and coordinate referrals as needed.
  • Ensure compliance with infection control and regulatory standards.
  • Participate in on-call responsibilities as assigned.
  • Attend interdisciplinary meetings and perform other related duties as assigned.

Required Qualifications

  • Current State Registered Nurse (RN) license.
  • Associate Degree in Nursing (ADN) or Nursing Diploma.
  • Current CPR certification.
  • Homecare Homebase (HCHB) experience required.
  • OASIS experience required.
  • 1–2 years of RN clinical experience required.
  • 2+ years of home health experience required.
  • Valid driver's license, reliable transportation, and insurance.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • CHPN certification.
