Description

Physical Therapist (PT) – Home Health

Location: Flint, MI
Contract Length: 14 Weeks
Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Shift: 8 Hours

Position Summary

We are seeking an experienced Physical Therapist (PT) to provide skilled home health services to patients in Flint, MI. The PT will evaluate patients, develop individualized treatment plans, deliver therapeutic interventions, and collaborate with physicians and the interdisciplinary care team to improve mobility, restore function, and enhance quality of life.

Key Responsibilities

  • Perform comprehensive patient evaluations and assess functional needs.
  • Develop, implement, and update individualized Plans of Care (POC).
  • Provide physical therapy treatments for neurological, orthopedic, and neuromuscular conditions.
  • Accurately document evaluations, progress notes, physician orders, and other clinical records.
  • Educate patients and caregivers on therapeutic exercises, mobility, home safety, and disease management.
  • Train patients in the safe use of assistive devices such as walkers, wheelchairs, canes, braces, and prosthetics.
  • Coordinate patient care with physicians and the interdisciplinary healthcare team.
  • Maintain compliance with infection control practices and regulatory standards.
  • Travel locally to provide therapy services in patients' homes. 

Required Qualifications

  • Current State Physical Therapist (PT) License.  
  • Bachelor's Degree in Physical Therapy from an APTA-approved program.
  • Current CPR Certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • 1–2 years of Physical Therapist experience in an acute care or rehabilitation setting.
  • Homecare Homebase (HCHB) experience required.
  • Ability to travel locally for home visits. 

Preferred Qualifications

  • Previous Home Health experience. 

Work Environment

This is a field-based home health position requiring local travel to patients' homes throughout the Flint, MI area. The role involves independent patient visits, collaboration with an interdisciplinary team, and the ability to lift, bend, push, pull, and stand for extended periods.
