Position Overview

We are seeking a qualified Licensed Master Social Worker (LMSW) to provide behavioral health social work services in an acute care setting. The Social Worker will assess patients’ psychosocial needs, provide therapeutic interventions, coordinate treatment and aftercare plans, and support safe and appropriate discharge planning.

The LMSW will serve as the primary therapist for assigned patients and collaborate with patients, families, physicians, and multidisciplinary healthcare teams to address mental health and psychosocial needs.

Key Responsibilities

• Conduct comprehensive psychosocial assessments at admission and throughout treatment.
• Identify and prioritize patients’ psychosocial problems and develop individualized treatment and aftercare plans.
• Serve as the primary therapist for assigned patients.
• Provide therapeutic interventions and evaluate patient responses to treatment.
• Collaborate with patients, families/significant others, physicians, and multidisciplinary healthcare professionals.
• Communicate significant social and emotional factors affecting patients’ mental health to the healthcare team.
• Coordinate and manage discharge planning for assigned patients.
• Arrange appropriate follow-up services and community resources.
• Advocate for patients when coordinating with hospitals, community agencies, and outside organizations.
• Maintain accurate and timely documentation in the medical record.
• Utilize EPIC for clinical documentation.
• Maintain professional competency, ethical standards, and ongoing professional development.
• Participate in special projects and community meetings as requested.

Requirements

• Current New York LMSW license required.
• Minimum 2 years of relevant social work experience.
• Behavioral health and/or acute care experience preferred.
• Strong assessment, therapeutic, discharge planning, and care coordination skills.
• Excellent communication and organizational skills.
• Ability to work effectively within a multidisciplinary healthcare team.
• Strong documentation and patient advocacy skills.
• Flu vaccine required — no exemptions or exceptions.
