Registered Nurse (RN) – Home Health

Location: Fayetteville, NC
Job Type: Contract
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekly Hours: 40 Hours
Productivity: 30 Points/Week
On-Call/Weekend: Yes
Mileage: Not reimbursed between visits
 

Position Overview

Cape Fear Valley Home Health is seeking an experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Robeson, Hoke, and Cumberland Counties. The RN will deliver compassionate, patient-centered care while coordinating services and ensuring accurate clinical documentation.

Key Responsibilities

  • Perform comprehensive nursing assessments and evaluate patients' health conditions.
  • Develop, implement, and update individualized patient care plans.
  • Provide skilled nursing care in the patient's home according to the plan of care.
  • Administer medications and treatments as ordered.
  • Perform wound care, medication management, and other skilled nursing procedures.
  • Monitor patient progress and identify changes in condition requiring intervention.
  • Educate patients and caregivers on medications, disease management, safety, and care plans.
  • Communicate effectively with physicians, therapists, caregivers, and other members of the healthcare team.
  • Complete accurate and timely clinical documentation.
  • Participate in care coordination and follow-up activities.
  • Manage a caseload while meeting the expected productivity of 30 points per week.
  • Provide on-call/weekend coverage as required.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Ability to provide skilled nursing care in a home health setting.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently and manage multiple patients effectively.

Preferred Qualifications

  • Home Health experience strongly preferred.
  • HCHB (Homecare Homebase) experience preferred.
  • Strong communication, organization, and time-management skills.
  • Comfortable traveling throughout the assigned coverage areas.
