Description

RN Case Manager – Simplified

Position: RN Case Manager
Shift: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours/week
Contract: 13 weeks

Requirements

RN Case Management experience: Minimum 2 years recent experience
Must have experience in:
- LTAC or STACH
- MCG
- Discharge planning in acute-care settings
- Strong collaboration with the Interdisciplinary Team (IDT)
Texas RN license or Compact/Multi-State RN license
BLS through American Heart Association (AHA) — must be attached to submission

Main Responsibilities

Assess patients and coordinate their care.
Create and implement care plans.
Coordinate discharge planning.
Ensure proper use of healthcare resources.
Work with doctors, nurses, social workers, and other healthcare professionals.
Advocate for patients and consider their individual needs and preferences.
Help improve patient satisfaction, continuity of care, and cost effectiveness.
Perform utilization review and case management.
