Description
Job Summary
We are seeking an experienced Licensed Master Social Worker (LMSW) to join a hospital-based Case Management team. The LMSW will play a critical role in coordinating patient care, facilitating safe and effective discharge planning, and ensuring patients receive appropriate resources and support throughout their hospitalization.
This position requires strong clinical assessment skills, collaboration with interdisciplinary teams, and experience navigating complex psychosocial and discharge needs in an acute care setting.
Responsibilities
- Conduct comprehensive psychosocial assessments for hospitalized patients.
- Coordinate discharge planning and transitions of care.
- Collaborate with physicians, nurses, therapists, case managers, and community agencies to develop patient-centered care plans.
- Assist patients and families with community resource referrals, financial assistance resources, and post-acute care arrangements.
- Provide crisis intervention, emotional support, and counseling as appropriate.
- Facilitate care coordination to promote quality outcomes and reduce barriers to discharge.
- Support utilization management and resource stewardship initiatives.
- Advocate for patients while ensuring compliance with regulatory and accreditation standards.
- Maintain accurate and timely documentation within the electronic medical record.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited program.
- Current LMSW license in the applicable state.
- Previous acute care hospital social work or case management experience required.
- Strong knowledge of discharge planning, care coordination, and community resources.
- Experience working with interdisciplinary healthcare teams.
- Excellent communication, organizational, and critical thinking skills.
Preferred Qualifications
- Hospital-based case management experience.
- Experience with utilization review or care management processes.
- Familiarity with electronic medical record systems.
- Background supporting high-acuity or critical care patient populations.
