LPN

• Every other weekend is a must and every other holiday.
• Floating to other units as staffing permits.
• May be needed to drop to CNA position as staffing permits and needs.

Unit Description:

• This is a 3-unit, 120 bed facility.
• Each unit has 40 residents max.
• This position is a 40 hr/5 evening per week position.
• The hours are 1500 to 2330.
• Staffing ratios for this shift are 2 LPN - 3-4 CNA for 40 residents.
• Typical orientation period will be 1 day on the assigned unit.
• Scrubs of any color may be worn by the employee.

Job description:

• The primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift.
• Responsible for meeting the established standards of skilled nursing practice.
• The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients.
• Effectively provides Person Centered care and services.
• Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
• Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditions
• Practices ethically and professionally within their scope; advocates for patient
• Maintains clinical competence specific to their care area including organization required education within stated timeframe
• Coordinates care with interdisciplinary team
• Documents appropriately and thoroughly in EMR/Point Click Care.  Prefer experience in Point Click Care.
• Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations
