Location: Richmond, KY
Job Type: Travel Contract
Schedule: Day Shift
Weekly Hours: 32 hours/week
Productivity: 25 points/week
On-Call: Minimum 1 day/week
Weekend: 1 weekend/month
Mileage: Reimbursed between patient visits
 

Position Overview

seeking an experienced Travel Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Richmond, KY service area. The ideal candidate will have strong Home Health and OASIS experience and be comfortable managing patients independently in a home-based setting.

Key Responsibilities

• Provide skilled nursing care to patients in their homes.
• Complete comprehensive patient assessments and develop individualized care plans.
• Perform OASIS assessments and documentation accurately and timely.
• Administer medications and provide treatments as ordered.
• Monitor patient conditions and identify changes requiring intervention.
• Provide wound care and other skilled nursing services as needed.
• Educate patients and caregivers regarding medications, disease management, and care plans.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Complete accurate and timely documentation in the EMR.
• Participate in required on-call and weekend coverage.
• Manage daily visits while meeting the 25-point weekly productivity expectation.

Required Qualifications

• Active RN license
• 3+ years of Home Health experience
• 3+ years of OASIS experience
• Strong clinical assessment and documentation skills
• Ability to work independently in a home health environment
• Willingness to participate in on-call and weekend rotation

Preferred Skills

• Homecare Homebase (HCHB) experience
• Strong OASIS knowledge
• Excellent communication and organizational skills
• Experience managing multiple patients and geographic areas
• Strong patient education and care coordination skills

Additional Details

• 32-hour weekly guarantee
• 25 productivity points/week
• Mileage reimbursement between patient visits
• Minimal on-call requirement of 1 day/week
• 1 weekend/month
• Day-shift position
• Royal Blue or Black scrubs
